ProviderSignal one-time file license (version 2026-09)

These terms come with every one-time file, inside the ZIP as README-and-license.txt, and apply to the organization that bought it. Version 2026-09.

1. What you bought. A snapshot of the standardized Medicaid fee schedule named in this file's README, in the edition shown there. States republish on their own schedule. ProviderSignal does not update a purchased file, though your download link serves the latest edition for as long as the link is valid.

2. Who may use it. One organization: the company, practice or agency that bought it. Anyone inside that organization may use the file for its own business, and the file may be loaded into that organization's own systems.

3. What you may not do. Resell, republish, sublicense, post, or otherwise make the file, any substantial part of it, or a table derived from it available to anyone outside your organization.

4. What is public and what is ours. The rates are public records published by each state's Medicaid program. The standardized layout, the plain-language code labels, the Medicare comparisons and the change history are ProviderSignal's compilation and are covered by this license.

5. No warranty. Rates are reproduced from each state's publication as of the verified date in the README. The file is not advice on coverage, billing or reimbursement, and ProviderSignal is not responsible for decisions made with it.

6. No refunds after download.
