New Dental Practice Registrations by State, 2021 to 2026

19,761 dental practice entities registered since 2021 across the 50 states and Washington DC, 11,730 of them at a building with no dentist before, 3,731 so far in 2026. One CSV per state and year, with the practice, its specialty, address, phone, the official on record and the verified website. Registered means the day the federal NPI registry issued the practice's number.

Edition 2026-11-02, from the NPPES monthly file; the files rebuild monthly.

Prices

Single-organization license, internal use, no redistribution. No refunds after download. Read the license. Secure checkout by Stripe; the link arrives by email and works 5 times in 30 days.

By state

Registrations on file since 2021, then this year's. A state opens the picker with its every-year bundle selected; the smaller link is the state's live page with the newest 90 days.

What a row is

One dental organization NPI the federal registry issued in the state during the year: the PLLC, LLC or group entity a practice files when it forms, with the day it was issued. Two kinds of registration are set aside because they are not new sites: a batch of three or more entities registered the same day under one official's phone (a restructuring), and an entity a year or more younger than an organization at the same building that shares its phone, its official or its name (a successor entity).

Every remaining row carries whether a dentist was already registered at the building before the entity: an existing practice taking an entity, not a new site. It is flagged, never filtered, so the choice is yours.

What the file is not

A list of closings. NPPES dates a registration and nothing else: a practice that closes or moves keeps its NPI, so no count here is a count of practices that closed, and the file never says net growth. A registration is not a guarantee the doors opened either: a few never become practices, and a practice can open months after its number is issued.

The phone, the official on record and the website are today's record, not the day's: a practice that changed its phone since registering shows the current one.

Questions before you buy

What counts as a new dental practice registration?
A dental organization NPI issued by the federal NPI registry (NPPES) in the state during the year: the PLLC, LLC or group entity a practice files when it forms. The file sets aside two kinds of registration that are not new sites: a batch of three or more entities registered the same day under one official's phone (a restructuring), and an entity a year or more younger than an organization at the same building that shares its phone, its official or its name (a successor entity). Every remaining row carries whether a dentist was already registered at the building before the entity, so you can tell a new site from an existing practice taking an entity.
Does the file say which practices closed?
No. NPPES dates a registration and nothing else: a practice that closes or moves keeps its NPI, so no count here is a count of closings, and the file never says net growth. It is the list of practices that registered, with the day they did.
What is in each row?
The registration date and year, the practice name, its dental specialty in plain words, the street address, city, state and ZIP, and from today's record the practice phone, the authorized official named on the registration (often the dentist behind it), the website ProviderSignal verified by fetching it, the dental support organization where the registry backs it, the existing-dentist flag, and the ten-digit NPI. 14 columns, the same layout in every file.
How current is it, and how far back does it go?
Years through the last yearly NPPES file come from the yearly files; the current year comes from the monthly file, so a year is complete once the next January file is in. The files go back to 2021 and rebuild from each monthly load; the edition date is printed on every tile and in the README. A download link serves the latest edition while it is valid.
Can I see a sample before buying?
Yes. Pick the file and enter an email: a sample spread through the file arrives in a minute, with the practice, specialty, town and date but with the phone, official on record, website and NPI held back. The full file carries them.
What does the license allow?
Single-organization license, internal use, no redistribution. No refunds after download. The full terms are in the ZIP as README-and-license.txt and at providersignal.com/files/license.
